CSB updates investigation into deadly chemical leak at PEMEX Deer Park refinery
The U.S. Chemical Safety and Hazard Investigation Board (CSB) updated its investigation into a deadly chemical leak at the PEMEX Deer Park Refinery. In October, about 27,000 pounds of hydrogen sulfide gas were released, killing two contract workers. The CSB found that the leak occurred when two workers partially opened a flanged connection on piping containing the toxic gas. One worker died at the scene, while another, working nearby, also succumbed to exposure. A lawsuit has been filed by the family of one victim, alleging negligence by PEMEX, Shell USA, and PMI Services.
